Hospice Medical Director
Confers with, and advises, the hospice team to oversee the plan of care.
Skilled Nurse
Registered and vocational nurses assess and manage symptoms and problems, whether related to the terminal illness or a coexisting condition. They make regular visits and teach caregivers how to effectively provide care and comfort. The focus is symptom relief through a comprehensive approach. Some of the skills and duties include:

Management of medications, IV therapy, wound care (including pressure sores)

Treatment of pain/discomfort, shortness of breath, urinary tract infections

Assessment for supplies and equipment needed (ie. hospital bed, air mattress)

Development of a patient/family-centered Plan of Care
Spiritual Counselor
Provides spiritual and emotional support to patients and families of all faiths as they desire.
Bereavement Counselor
Provides support during hospice care and for 13 months after death. Services can range from crisis intervention, to private and group grief support, or family/individual counseling.

Attending Physician
May remain patient's primary physician if the patient, family, and physician desire. The MD will participate in the plan of care.
Hospice Aide
Provides personal care and assistance with hygiene and other activities of daily living.
Social Worker
Cares for the psychosocial needs of the patient and family facing terminal illness. Coordinates services with physician and team and assists with community resources. Available to guide with legal, financial, and social services.
Dietician, Physician, Occupational Speech, and Other Therapist
These services are available by consultation as needs are identified.
